Introduction

You are about to start an online nomination for the Adult Learners' Week Project/Provision Awards 2015. To ensure that you understand the process, please read the instructions below:

  • Before beginning to complete the nomination form, please read the Guidance Notes and Eligibility Criteria.
  • The nominator will be asked to complete Parts 1, 2, and 4, and the learner will be asked to complete Parts 3 and 4. Providing this information will help us to gain a fuller picture of the learning undertaken. All parts of the form need to be completed by the deadline, 5pm Wednesday 17 December 2014.
  • The nominator's section must be completed first. Once the nominator's form is submitted, an e-mail will be sent to the learner (to the e-mail address provided in Part 3). In the email the learner will receive a unique link for them to access their sections of the form, if this isn't received within 2 hours please contact the Adult Learners' Week Team
  • If you are unable to complete the form in one go, the 'Save & Continue' button will allow you to come back to the form at a later time. Please ensure that you save your work regularly as you complete each section, the 'Save & Continue' function will only work once a section is complete.
  • When you click on the 'Save & Continue' button for the first time, an email will be sent to you with a link to your nomination. Please keep this email safe so you can access your nomination at any time. If you do not receive a link to the nomination form please contact the Adult Learners' Week Team.
  • Only the information contained in the form can be used when considering the nomination. Please do not send any additional support information/materials.

Part 4: Declaration

As we use the information that has been supplied in this form to inspire others to take up learning, we want you and those involved in your project/provision to be completely happy with what has been written on this form.

Award winners' often attract interest from local, regional and sometimes national press and media. For this award this may include having a profile written about the project/provision, having a photograph taken for inclusion in the winners' brochures and other publicity materials, being filmed for a video about the learners' learning journey and/or being interviewed by the media for publicity. We normally invite national and regional winners' to attend an awards ceremony where they will be presented with their award, invite them to tell us very briefly about their learning journey and we may also take photographs.

Please read this form very carefully and if there is anything at all that you as the nominator do not wish to be made public, please let us know in the box below. Please also indicate if you or any of the learners who take part in the project/provision do not wish to have their photograph taken or be included in a video.
